The Pharmacy
The
Hospital Pharmacy in Hong Kong Adventist Hospital provides
a full-range of pharmacy services to both inpatients
and outpatients. The scope of our services also extends
to provide medication counseling to patients upon discharge,
reconstitution of chemotherapy drugs and TPN service.
Our policy & procedure ensure timeliness and accuracy
in the services we provide. In-patient rounds are made
at least hourly, chemotherapy drug therapies are provided
within 1 hour, and TPN are compounded daily.
The level of care provided by our services meets the
patients' needs through drug therapy monitoring, counseling
and other clinical interventions, that are designed
to ensure effective drug use and minimize potential
risks of drug therapy.
Below are several pharmaceutical
systems adopted here at Hong Kong Adventist Hospital
to promote safe and rational use of drugs.
| Unit Dose Drug Distribution

The Unit Dose Drug Distribution system has been
adopted for inpatient dispensing since 1983. The
system provides the backbone for pharmacists to
follow drug treatment during a patient's hospital
stay. Charts are constantly reviewed by pharmacists.
Appropriateness of drug therapy is monitored daily.
Systems are in place to double check on our performance
through surveys and feedback. This ensures that
the right drug is given to the right patient at
the right dose and at the right time via the right
route of administration. As a result, incidences
of medication error have been kept to a minimum.

| Bedside Discharge Drug Counseling
service |

The objective of Bedside Counseling is to inform
patients, in the comfort of their own rooms, on
the proper use of their medications. This service
promotes drug compliance, alerting patients of
the potential side effects of drugs, and helping
to prevent common food-drug or drug-drug interactions.

| TPN service |
 |
|
We started to provide Total Parenteral Nutrition
(TPN) service since 2000. As the name implies,
TPN is an intravenous therapy that provides nutrition
to patients who could not take any nourishment
by mouth. To ensure quality and sterility of the
TPN preparations, the admixing procedure is performed
in a laminar flow hood installed in the clean
room under strict aseptic techniques.

| Cytotoxic Reconstitution Service |
 |
| Cancer patients often have lowered immunity against
common infections. The maintenance of sterility
is of paramount importance in the preparation and
reconstitution of chemotherapeutic agents. We employ
strict isolation technology in our cytotoxic reconstitution
service so as to safeguard the health of all clinical
personnel and protect the environment. |
