Eligibility
- Hong Kong Permanent Resident aged 60 or above
- Enrolled in eHealth with sharing consent granted to the Hospital
- In stable medical condition (medical history report required for patients on long-term medication)
- Currently on the Hospital Authority (HA) waiting list, OR holds a referral letter from a cardiologist certifying the need for treatment
- Passes financial assessment OR currently receiving Comprehensive Social Security Assistance (CSSA)
